(1 per person on roster) - Copy of birth certificate, passport or other legal proof of age (1 per player)
- Copy of photo ID (1 for each individual on the roster--staff and players); i.e. Passport
- A letter of good standing from your national volleyball federation with your team entry and check.
- $100 check for mandatory sport accident insurance
