Additional Required Forms (Non-USA Teams Only):
- 2017-2018 Foreign Team Participation Summary Form - (1 per Team)
- 2017-2018 Foreign Participant Registration Form - (1 per Individual on the roster)
- Copy of birth certificate, passport or other legal proof of age - (1 per player)
- Copy of photo ID (1 for each individual on the roster); i.e. Passport
- A letter of good standing from your national volleyball federation with your team entry and check.
- $100 check for mandatory sport accident insurance
